New Patient Instructions

Start your journey towards precision health. We provide streamlined scheduling options to ensure you receive timely and accurate clinical care.

Step 1

Request an Appointment

You can request an appointment online (click here) or by calling 408-356-4242. Online requests are recommended to avoid wait times.

Indicate Priority: If your condition requires timely attention, please indicate the level of priority when submitting your form or calling our office, or have your referral physician send a note to our office.

Step 2

Complete Your Pre-Visit Check-In

Once your appointment is confirmed, you will receive a text message from our office. Follow that link and complete your check-in process prior to your appointment. Check-in includes filling out your questionnaire and signing consent forms.

Step 3

Stay Connected

After your visit, use the patient portal or Healow app to communicate with our office, request refills, view results, pay bills and more. Download Healow app on your mobile phone. After your first visit, you can also use patient portal if you are using a desktop.

If you have difficulty with the online check-in process, you can also download the new patient intake form on this website (Click Here), print it, complete it and then bring it to the office at the time of your appointment to save time.
